PDBR Application Process

To be eligible for the Physical Disabiltiy Board of Review (PDBR) you must have:

  • Been medically separated between September 11, 2001 and December 31, 2009
  • A combined disability rating of 20 percent or less
  • Not been found eligible for retirement

Answer the following questions to help determine if you meet the criteria:

  1. Were you permanently separated from military service after September 10, 2001 but before January 1, 2010?
  2. Was the reason for your separation from military service due to a disability?
  3. Was the disability level awarded by your military department at the time you were permanently separated for your disability less than 30% combined?
  4. At the time you were permanently separated from your military service by reason of disability, you were not otherwise eligible for retirement from your military service.

If you answer 'yes' to all of these questions, you are eligible to apply to the PDBR to have your disability rating determination at separation reviewed.

If you can't answer yes to all of these questions, then you're not eligible to have your disability rating determination reviewed by the PDBR. However, you should consider other options for review, including your military department’s board for correction of records.

Applying for the PDBR

  1. Download the DD Form 294 and complete it per the instructions on page 3.
  2. Send completed form even if you do not know or have all the information to the PDBR.
  3. Include a copy of your Certificate of Release or Discharge from Active Duty, DD Form 214 or DD Form 215 (correction). Note, please mail us the DD Form 294 even if you don't have a copy of your DD Form 214 or 215:

  • Please ensure the documents are scanned in at a high enough resolution that we can run text recognition. Any pictures or images should be scanned at as high a resolution as possible. Unreadable information could delay your case due to the need to request legible paper copies.
  • Our ability to accept email is limited to 1 GB of data per email.  Please break up the file size accordingly.
  • A scanned or facsimile copy of the application form is considered a legitimate application.
