The Office of the Assistant Secretary of Defense for Health Affairs (ASD-HA) in coordination with Defense Health Agency (DHA) is sponsoring this survey.
The purpose of this study is to assess health behaviors that have the potential to impact readiness and to assess the overall well being of the Armed Forces. The Department of Defense (DoD) uses data from the Health Related Behavior (HRB) Survey to improve education, training, treatment, and counseling to support the Services and optimize individual and overall health status and fitness.
This is an anonymous survey. If you are selected to participate, please be as accurate and honest as possible. The questionnaire contains items about diet, exercise, drug, alcohol, tobacco use, gender-specific issues, oral health, safety and deployment.
The DoD has been conducting this survey since 1980, the current 2011 survey is the 12th iteration. The survey will be administered on-site at selected installations and for the 1st time via the internet.
TMA/Defense Health Cost Assessment Program Evaluation has contracted with ICF International to conduct this survey.
