- Height
- Weight
- Blood Pressure
- Vision Screening
- Provider (PA/NP) interview
Service member/Command leaves PHA Event process with all PHA documents for review by state medical personnel
- The vendor is responsible for
- Signature of the PHA in the MHA module
- Upload of supporting documentation in HRR
- The state is responsible for initiation of profile in e-Profile

- Copy of focused exam (if performed)
- Referral recommendation form
The vendor is responsible for:
- Suggested profiling actions documented on draft DD Form 3349 (no e-Profile entry)
- Signature of the PHA in the MHA module
- Upload of the PHA questionnaire and supporting documentation to HRR
