An organized effort aimed at achieving desired health outcomes in populations with prevalent, often chronic, diseases from which healthcare delivery is subject to considerable variation. Disease management (DM) programs use evidenced-based guidelines, to provide information and tools in promoting self-management activities with Military Health System (MHS) Quadruple Aim goals of increasing patient and provider satisfaction and improving clinical and financial outcomes, while advocating the appropriate utilization of resources.
