Definition:

A healthcare professional that uses a collaborative approach in identifying patient and disease specific education and care needs, to provide comprehensive, patient-centered, proactive strategies promoting prevention, health, and wellness concepts that support the provider’s Plan of Care. In the direct care system (DCS), the disease manager delivers disease management (DM) services to Prime enrolled TRICARE beneficiaries at the military treatment facilities (MTF). The DM specialist in purchased care system (PCS) provides individualized, one-on-one telephonic education and resource health information services designed to empower the beneficiary to better self-manage their disease and communicate with their provider.
