A complete history, review of systems, and physical exam performed by the DoD as part of the Separation Health Assessment (SHA) Program in accordance with the standards set forth in the 2013 VA/DoD memorandum of agreement (MOA). It is intended to meet the statutory and DoD requirements for a separation exam. The Service member input is documented on DD Form 2807-1 and the physical examination and physician assessment on DD Form 2808 or equivalent electronic medical record templates.
