A health assessment tool used to evaluate and document a Service member’s medical condition, medical readiness status, and deployability status. It is the cornerstone of force health protection, as it is an opportunity for medical providers to identify medical concerns, educate Service members as to their medical conditions, and refer Service members for further care, as indicated.
