Care Management Team (CMT)

Definition:

Individuals who are working together to manage, coordinate, or deliver the care, benefits, and services for the recovering Service member (RSM) and to support the RSM’s family. The professionals and individuals who comprise a specific CMT will vary based on the needs of the individual and their family (e.g., health care provider(s), nurse case manager, therapist, social worker, chain-of-command representative, transition coordinator, and all others providing care, benefits, and services), but at minimum, will consist of a medical case manager and a non-medical case manager, such as an RCC.
