Partnering with providers to ensure seamless transitions and continuity of mental health care for patients
Whether a patient is relocating, returning from deployment, changing duty stations, or preparing to leave military service, inTRANSITION helps bridge the gap and supports continuity of care. Healthcare providers, care teams, and community partners play a critical role in connecting people to the program.

To support providers and military organization outreach, DHA offers free briefings about inTRANSITION for healthcare teams, providers, military and veteran support organizations, and other partner groups. Web-based presentations, video teleconferences, and onsite briefings are available upon request.

Resources
Access inTRANSITION’s downloadable and printable materials. Program brochures, wallet-sized information cards, and more can be downloaded or ordered worldwide free of charge. Orders are typically shipped within 7 days of receipt, though may take up to 15 days to be received.
How to Order Printed Materials.
Please email your request for materials and specify:
